Person Centred Planning (PCP) is an approach that places the person at the centre of service planning, design and delivery. PCP is driven by the principles of self determination, choice and self advocacy.

PCP seeks to identify and provide services based on people's personal aspirations and dreams. Person directed planning involves a holistic approach, considers all aspects of the person's life and understands that achieving personal goals occurs in a social and community context.


The Person Centred Planning process involves:
  • Painting a portrait of whom the person really is and what they want to do with their life
  • Identifies and includes people that are important in the person's life to be part of the process, such as:
    • family/whanau,
    • friends
    • support persons
    • other professionals
  • Enhancing people's life experiences and encouraging them to dream and realise a better life for themselves
The person centred approach is 'strengths' based, focusing and building on peoples abilities and potential, rather than their disability.

Focus 2000 is using the Personal Outcomes approach to defining service quality. Personal outcomes are important because they put listening to and learning from the person at the centre of organisational life, and provide a guide to person directed planning.
